Snapping hip is a condition where you feel or hear a snapping, clicking, or popping sensation in your hip, especially during activities like walking, standing up, or sitting down. This condition is also commonly referred to as a popping hipor clicking hip.

There are three main types of snapping hip:

  • External snapping hip syndrome

  • Internal snapping hip syndrome

  • Intra-articular snapping hip syndrome


External and internal snapping hip syndromes are typically diagnosed clinically through specific physical examination maneuvers. In contrast, intra-articular snapping hip syndrome usually requires imaging tests such as an MRI to confirm the diagnosis.


Treatment varies depending on the underlying cause and the duration of symptoms. In most cases, non-surgical treatment options—such as rest, stretching exercises, physical therapy, and activity modification—are effective in relieving symptoms.


However, if the snapping is due to an intra-articular issue (such as a labral tear or loose body inside the joint), hip arthroscopy surgery may be necessary.


What Causes Snapping Hip Syndrome?

Snapping hip syndrome occurs when a muscle or tendon moves across a bony prominence around the hip joint. This movement creates a snapping or clicking sensation. While this condition is often painless and harmless, some people find the sound uncomfortable or concerning. In some cases, the snapping can lead to inflammation of nearby bursae(fluid-filled sacs around the joint), resulting in pain.


External Snapping Hip – Lateral Hip Clicking

The most common type of snapping hip involves the iliotibial band (IT band) snapping over the greater trochanter, a bony part of the femur on the outer side of the hip.When the hip is extended, the IT band is located behind the greater trochanter. As the hip flexes, the IT band moves forward. Because the IT band is naturally tight like a stretched rubber band, it can make a snapping sound as it shifts back and forth over this bony prominence.


Internal Snapping Hip – Front of the Hip

Another cause is the rectus femoris tendon, which can snap over the head of the femur when the hip moves. When you flex the hip, the tendon glides over the front of the femur; as you extend the hip, it slides back. This back-and-forth motion can result in an audible snapping sound.In addition to the rectus femoris tendon, the iliopsoas tendon can also snap over the pelvic bone, contributing to internal snapping.


Intra-articular Snapping – Inside the Joint

Sometimes, the snapping sound originates from within the joint itself due to labral tears or cartilage injuries inside the hip. These causes are typically painful and may require further imaging and advanced treatment.


Clicking Sound After Hip Replacement Surgery

After hip replacement surgery, some patients report hearing clicking, popping, or snapping sounds coming from their new hip joint. These sounds may be due to a variety of causes—some completely normal, others potentially concerning.


Common and Usually Harmless Causes Include:

  • Soft Tissue Movement: Tendons and muscles may shift and occasionally rub against the prosthetic components, producing clicking sounds. This is often temporary and harmless.

  • Synovial Fluid Motion: The movement of synovial fluid—natural joint lubrication—within the artificial joint can produce clicking, especially during position changes or movement.

  • Scar Tissue Formation: Some scar tissue or adhesions can form after surgery and may cause snapping or clicking as the hip moves. Scar tissue is a natural part of healing and is not always problematic.

Less Common but Important Causes:

  • Prosthetic Component Issues: Rarely, a popping or grinding sensation may result from loose or misaligned implant components. If this is the case, the noise is often accompanied by additional symptoms.


Watch for These Warning Signs:

  • Persistent pain or discomfort in the hip, groin, or thigh

  • A feeling that the hip is giving out

  • Difficulty bearing weight or walking on the affected leg

  • Limited range of motion or stiffness in the joint


If the clicking or popping sound is not associated with pain or loss of function, it is usually not a cause for concern. However, if pain is present along with the sound, it should be properly evaluated by your orthopedic surgeon.
